For class at the beginning of the year I had do a tonal painting and it had to be a profile picture. Since we were focusing on doing a portrait and grey tones for the picture, I didn't add in a background or the cigarette smoke coming out of the mouth and going onto the top lip as seen in the reference picture.
Done in PS CS6 with an intuos4.
